新生儿并发症患儿的远期健康结局:流行病学视角与方法学进展
Long-Term Health Outcomes Among Offspring with Neonatal Complications: Epidemiological Perspectives and Methodological Advances
特刊简介:
近二十年来,高收入国家新生儿存活率显著提升,极早产儿(孕周<32 周)与极低出生体重儿(出生体重<1500 克)的存活情况尤为明显。但这些存活患儿在儿童期及青少年期发生严重神经发育障碍的风险极高,且风险随出生胎龄降低而上升。目前,针对早产儿、足月儿及合并复杂新生儿疾病患儿的远期预后,仍缺乏大型当代人群队列研究。在探讨产前与新生儿期因素对精神健康结局的远期影响方面,仍存在诸多研究空白。
其一,相关因果通路的量化研究尚不充分,部分原因是人群研究中对遗传易感性的校正不足。
其二,基于家庭的研究设计(如同胞对照研究)有助于区分遗传与环境因素的作用。
其三,早产的远期后遗症可能因早产亚型而异(自发性早产与医源性早产),但鲜有研究探讨这一差异。
其四,远期研究中的失访偏倚问题突出,失访儿童往往健康结局更差,易导致结果出现偏差。
本期特刊议题包括但不限于:
Design and methodological considerations;
Casual inference approaches in life-course epidemiology;
Use of sibling and family-based design to control for unmeasured confounding;
Addressing time-varying confounding in studies of early-life exposures and later outcomes;
Analytical approaches for predicting long-term outcomes based on neonatal conditions;
Empirical associations;
Neurodevelopmental and cognitive sequalae of adverse neonatal conditions;
Physical and chronic health sequalae of adverse neonatal conditions;
Epigenetic programming following adverse birth exposures;
International comparisons of neonatal networks and harmonized registries: neonatal outcomes, long-term follow-up, and methodological pitfalls.

母亲年龄:对围生期与儿童健康的影响
Maternal Age: Consequences for Perinatal and Child Health

自 2016 年起,美国 30 多岁女性的生育数量已超过 20 多岁女性。生育年龄推迟是过去 40 年间最重要的人口结构变化之一。
尽管从社会角度看,生育推迟通常被视为一种进步,但近期严谨研究证实,即使在 35 岁(“高龄妊娠” 分界)之前,高龄产妇(AMA)仍与不良围生期结局及儿童健康问题相关。该研究在校正了高学历人群推迟生育带来的强混杂因素后发现,高龄产妇对多种围生期与儿童结局产生的生物学影响,可能比以往认知更为显著。同期,青少年生育率降至历史最低水平,这意味着当前最年轻的产妇群体规模更小、特征也更为特殊。近年来数据可及性与研究设计的不断完善,以及对生物学衰老与实际年龄区分的深入探索,有望为揭示母亲年龄(无论偏低或偏高)与不良妊娠结局之间的因果关系提供全新见解。
本专题欢迎围绕母亲年龄与围生期、儿童健康结局展开的前沿研究,尤其关注阐明其内在机制的相关成果。
本期特刊议题包括但不限于:
Registry-based studies on AMA and perinatal and/or child outcomes (e.g., stillbirth, fetal growth restriction, chromosomal anomalies, developmental delays);
Novel study designs to control for strong confounding by high-education selection into older ages of childbearing;
Revisiting evidence that informs clinical guidelines, interventions (e.g., induction, cesarean delivery), or counseling among AMA populations;
Investigation of childbearing at early ages (i.e., teenage years), with careful consideration of factors by which persons select into teenage childbearing;
Use of -omics, placental, or other biomarker data to separate biological from chronological aging;
The social and biological implications of AMA;
Consideration of parity as a mediator, effect modifier, or confounder;
Comparative work on compositional changes in groups who are more recently delaying childbearing but who experience structural disadvantage (e.g., immigrant populations);
Comparative analyses leveraging the full maternal age distribution to contextualize risks observed at both younger and older ages within the broader continuum;
Methodological advances that may disentangle the relative contributions of AMA, advanced paternal age, and parity;
Careful assessments of rates of aging across subgroups, and/or examination of AMA/outcome disparities;
The role of assisted reproductive technology, and other proposed antecedents, on rising AMA across cohorts and/or over time.

《公共卫生与流行病学》(PPE)诚邀投稿本期关于 非必要剖宫产(Unnecessary Caesarean Delivery, CD)的专题。我们征集旨在明确相关定义与测量方法、测算发生率及影响因素、评估对母婴的短期与远期健康结局,并分析非必要剖宫产卫生经济学影响的研究。本刊尤其欢迎基于行政数据与登记数据、采用严谨因果推断方法的研究成果。
本期特刊议题包括但不限于:
Defining "unnecessary CD" in different health care contexts from clinical, and epidemiological perspectives (cross-country comparability); estimating rates and associated maternal characteristics (clinical and socioeconomic).
Methodological challenges in defining and studying unnecessary CD using health administrative/registry data (measurement error, coding validity, confounding, causal inference).
Maternal outcomes following unnecessary CD: short- and long-term physical, mental, reproductive, and obstetric sequelae.
Offspring outcomes following unnecessary CD: short- and long-term neonatal, pediatric, developmental, and psychosocial outcomes.
Interventions to reduce the rate of unnecessary CD such as adherence to evidence-based labor management protocols, continuous labour support, TOLAC promotion, benchmarking, midwifery-led care models, patient education-based interventions/decision-making tools, antenatal anxiety reduction interventions.
Health-economic implications of unnecessary CD: costs, cost-effectiveness of interventions to reduce overuse, budget impact, and equity considerations.
